自清雍正以来，乐家老店同仁堂被钦定为“御药供奉”，至乐家第八代已声名大震。病家闻名纷纷求药，而日本浪人小野勾结在乐家长大的曹化伦，蓄谋夺走密方药谱。曹化伦盗方被老东家乐宏达撞见，行凶纵火，烧毁乐家细料药库；又勾结御药房白太监扣押御药银子，竟使老店雪上加霜，无钱制药，无药可售。王爷误以为乐家“拿搪”，一怒下令撤销乐家御药供奉，另行招标。面对内外交困，乐宏达老人为保住二百年老字号，毅然隔代传印，将堂印传给孙子乐宏达。乐宏达临危受命，在妻子徐瑞珍帮助下，以海底捞月之功，夺回御药供奉。并得到光绪皇帝的赐匾。曹化伦又怂恿王爷，下令老店不得在民间售药，只供宫廷。乐宏达体味到“进则为良相，退则为良医”，“进而济世，退而济民”，“抗旨”卖药。乐宏达力助孙子，利用大总管李莲英的病情，一药回天。西太后终于开禁。流落北京被乐家收留的女伶丛玉珠，暗恋乐宏达，然而为营救乐宏达出狱，舍身将自己嫁给曹化伦。甲午战争，清军赴朝鲜抗敌。乐宏达赶造出十万包“诸葛行军散”，赠送将士，却被药材商人借机抬价，欠下巨款。年关债主逼债，乐宏达借贷无门，愤而变卖祖宅。药材商人得知赠药原因……
