明代中叶，御医李鹤龄和金衣卫杨傲是生死之交，杨傲爱上李鹤龄未婚妻霓裳，为此，栽赃嫁祸鹤龄奸了慧妃。为救李鹤龄，霓裳下嫁杨傲。他知道霓裳嫁杨傲后，隐姓埋名到山西原仓县开了医馆，自称「金胡子」。十二年后，鹤龄在查其弟子徐来风姐姐的案时，阴差阳错的帮了新知县双胞胎弟弟刘福星的忙。福星因立大功被召上京，要求金胡子同往。京中因一大案，鹤龄再次面对杨傲，杨傲也肯定了刘福星身后的「金胡子」就是李鹤龄，再次设计陷害,在调查一巨富杀人案中，金胡子及福星再次携手，查出真凶是杨傲的手下范泰的，范泰为求赎罪，说出当年杨傲陷害鹤龄的阴谋。金胡子上京为自己平反，与福星联手与已成为“定国大将军”的杨傲展开了斗争，杨傲拥兵自重要挟皇上。为了成全金胡子，霓裳使计调走杨傲的百万大军，杨傲一怒之下将霓裳杀害。金胡子找到杨傲，二人大战。金胡子用金针找到他的罩门并将他置于死地，为自己冤案平反。李鹤龄谢绝了在宫中当首席御医，福星放弃了迎娶郡主的机会，李鹤龄、刘福星、徐来风、辣椒一同再度笑傲江湖……
